G-STAR SCHOOL ADMISSION FORMS ARE COMPLETELY SEPARATE FROM THE MAGNET PROGRAM ADMISSION FORMS. YOU MUST FOLLOW THE INSTRUCTIONS BELOW TO OBTAIN THEM.

The student Admissions Forms can now be downloaded from this site here.

DIRECTIONS TO DOWNLOAD ADMISSIONS FORMS:
1. Click "here" to download the admission forms.
2. A screen will appear asking you to save or open the file.
3. Select open and the file will open directly in your web browser
4. Once the document is open then go to the top of your screen and click on print just as you would print any other document.


You CAN also obtain copies by:
(1) contacting your school 8th grade guidance counselor or,
(2) E-MAIL us below with your request and we will be happy to e-mail the Admission Forms to you DIRECTLY.

If you still cannot obtain an Admission Form, then please call 561.967.2023 and we will mail an admission form to you.

SPECIAL NOTE TO BAK MIDDLE SCHOOL OF THE ARTS STUDENTS:
